Small-town ER doc: "Yeah, I have a transfer for you."
Me: "Why do they need to be transferred?"
ER: "Well, they have a pneumonia."
Me: "Are they intubated?"
ER: "No. Their sats are normal on room air."
Me: "Then why do they need to come to us? You guys are a hospital, right?"
ER: "Yep. Their primary care physician didn't want to round on the patient so he demanded we transfer."

Primary care Doc needs to be fired, but he is likely the only doc anywhere around.
I was asked to have a patient transferred because of their "history of lung cancer".

I considered accepting just so I could tell the patient what his doctor said.
I once had a surgeon refuse to come to my hospital (where he had privileges) and transfer the patient to another hospital he liked better. It was just an appy. I was so pissed off, I sent him to the university mecca instead. He's part of my group (ED is just a side line) and it didn't really endear me to him.
