We don't get to see children much in the hospital, at least not where I work. I was trained family medicine, but now do mostly adult/critical care, so I don't get to take care of kids much anymore. It's a treat, then, when they are visiting their parents/grandparents, etc...
Me: "And who is this over here?" *point at ~5 year-old girl*
Mother: "That's my daughter."
Me: "She's very cute. How old are you?"
Kid: *holds up four, maybe five fingers* (coordination not the best).
Me: "Wow, you're getting big!"
Kid: "My mommy says 'a apple a day keeps doctors away'. Is that true? (this came out chwoo)"
Me: "Only if you throw them very hard at their head."
When I looked back up, the parents were glowering at me.

This is how inappropriate a grandparent I am, if you had said that to Jack, I would have laughed.
